Active Duty Military Community Survey

This form is for those at Peninsula Bible Fellowship and is simply designed to help us get you information about up and coming events for the military community! 

We ask for things like address, birthday, and marital status because we want to celebrate you, potentially send you invitations, and support your family when you are underway/ deployed.

If you are a spouse who isnā€™t sure, thatā€™s OK! Simply put ā€œnot sureā€.

The reason we are looking for this is just to help us get a better picture of your responsibilities (weight you carry), and to be prepared to send you off well when your boat goes underway/deploys.
